About the Role

Burke is seeking an Intake Coordinator (IC) to serve as the first point of contact for individuals seeking Intellectual and Developmental Disability (IDD) and Mental Health services. This role is essential in helping clients navigate the intake process and access appropriate care and services.

What You'll Do

• Serve as the initial contact for individuals seeking IDD and Mental Health services
• Provide information on available programs and explain the intake process
• Conduct intake interviews and Service Coordination Assessments
• Schedule appointments and coordinate access to services
• Manage referrals and connect eligible clients to appropriate IDD services
• Enter HCS/TxHml Interest List clients into CSIL system
• Obtain and track required documentation, including Releases of Information
• Maintain accurate and timely documentation in the EMR
• Ensure compliance with HIPAA and Burke confidentiality policies
• Perform additional duties as assigned
