Position Title: Mental Health Clinician - Angelina Mental Health Clinic

Location: Lufkin, TX
Schedule: Monday - Friday | 8:00 a.m. - 5:00 p.m. (occasional after-hours as needed)
Salary: $42,120 - $43,555 annually
Burke is seeking a dedicated Mental Health Clinician to support continuity of care for individuals transitioning from inpatient psychiatric treatment to outpatient services at the Angelina Mental Health Clinic. This role is essential in ensuring clients experience a seamless, well-coordinated transition to community-based care.
The clinician will obtain and organize clinical records, coordinate with inpatient and outpatient providers, and connect clients with appropriate outpatient services in a timely manner. This position requires strong organizational skills, effective communication, and the ability to manage multiple cases in a fast-paced environment while maintaining accurate and timely documentation.
Coordinate transitions from inpatient psychiatric facilities to outpatient services
Obtain and organize necessary clinical documentation
Communicate and collaborate with inpatient and outpatient treatment teams
Link clients to appropriate outpatient services and ensure follow-up appointments are completed
Identify and address barriers to care
Maintain accurate, timely, and compliant documentation
Manage multiple cases effectively with strong time-management skills
Education:
Bachelor's degree from an accredited college or university in psychology, social work, medicine, registered nurse, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, early childhood intervention, or another Human Services discipline that meets QMHP requirements.
Licensure/Certification:
Valid Texas driver's license
Driving record insurable by Burke's insurance administrator
Ability to complete SAMA, CPR, First Aid, and Defensive Driving training modules
At least six (6) months of full-time direct care experience in a mental health setting
Experience conducting mental health screenings and assessments
Ability to see, hear, and communicate effectively
Lift up to 50 pounds
Prolonged standing, walking, and driving
Hand and finger dexterity
