About the Role

Burke is seeking a Wrap Care Coordinator to support children and adolescents at risk of out-of-home placement. This role focuses on helping families build the skills and support systems needed to keep youth safely in the home while addressing behavioral health needs.

You'll work as part of a collaborative team alongside clinicians, families, and community partners-including the Department of Family and Protective Services and the Juvenile Justice System-to develop and implement individualized Wraparound treatment plans.

What You'll Do

  • Coordinate care and provide case management services for youth and families
  • Conduct assessments and support development of Wraparound treatment plans
  • Facilitate team meetings and collaborate with families and community partners
  • Help families build skills and access resources to prevent out-of-home placement
  • Provide crisis support and conduct emergency assessments as needed
  • Maintain accurate, timely documentation in compliance with Medicaid and agency standards
  • Use a strengths-based, family-centered approach in all interactions

What We're Looking For

Required:

  • Bachelor's degree in Social Work, Psychology, Rehabilitation, or related Human Services field (meeting QMHP requirements)
  • Valid Texas driver's license
  • Reliable personal vehicle and ability to travel within the service area
  • Ability to complete SAMA, CPR, First Aid, and Defensive Driving training
  • Insurable driving record

Preferred:

  • 6+ months of direct care experience with children or adolescents in behavioral health or education
  • Knowledge of child/adolescent growth and development
  • Familiarity with DSM-5 diagnostic criteria
  • LBSW certification

Work Schedule

  • Monday-Friday, 8:00 AM - 5:00 PM
  • Occasional overnight travel to Texas Residential Treatment Centers

Compensation & Benefits

  • Salary: $42,120 - $43,555 annually  
  • Travel reimbursement provided

Physical Requirements

  • Ability to lift up to 50 lbs.
  • Prolonged standing, walking, and driving
  • Manual dexterity, vision, hearing, and communication abilities
