About the Role
Burke is seeking an organized and compassionate Service Coordinator to join our Service Coordination team in Nacogdoches. This position provides ongoing direct support to individuals served through General Revenue Programs and the Community First Choice Waiver.
The Service Coordinator completes assessments, coordinates program planning and development, monitors services, connects individuals with appropriate resources, and provides crisis prevention and management. This position works closely with individuals, families and legally authorized representatives (LARs), team members, community resources, and internal and external providers to coordinate services and support individual needs.
This role requires strong documentation, organization, communication, and time-management skills. Regular travel is required to provide monitoring services in individuals' homes, schools, and/or day service environments.
What You'll Do
- Provide ongoing service coordination and direct support to individuals served through General Revenue Programs and the Community First Choice Waiver.
- Complete required assessments and documentation accurately and within established deadlines.
- Facilitate program planning and development based on individual needs.
- Monitor services to ensure appropriate support is being provided.
- Link and refer individuals to appropriate programs, providers, and community resources.
- Provide crisis prevention and management as needed.
- Develop an understanding of applicable reimbursement practices and program guidelines.
- Collaborate with individuals, families/LARs, team members, community resources, and internal and external providers.
- Travel to individuals' homes, schools, and/or day service environments to provide monitoring and coordination services.
- Maintain accurate, timely, and compliant documentation.
- Organize and manage responsibilities effectively to meet strict deadlines.
Qualifications
Required
- Bachelor's degree from an accredited college or university in Social Work, Rehabilitation, Psychology, Medicine, Nursing, Counseling, Sociology, Human Development, Gerontology, Educational Psychology, Criminal Justice, or another social, behavioral, or human services field.
- Proficient and timely documentation skills.
- Good typing skills and working knowledge of Microsoft Word.
- Strong organizational and time-management skills with the ability to meet strict deadlines.
- Ability to work independently and collaboratively with individuals, families, providers, and community resources.
- Valid Texas driver's license.
- Driving record that meets Burke's insurance requirements.
- Reliable transportation for routine travel throughout the service area.
- Ability to successfully complete Satori Alternatives to Managing Aggression (SAMA), CPR, First Aid, Defensive Driving, and other required training.
Preferred
- Experience providing service coordination, case management, or similar support to individuals receiving community-based services.
- Knowledge of community resources, program guidelines, and reimbursement practices.
Work Schedule
- Monday - Friday
- 8:00 a.m. - 5:00 p.m.
- After-hours work may be required to meet individual and program needs.
- Routine travel throughout the service area is required.

Physical Requirements
- Visual and auditory abilities are sufficient to evaluate and monitor individual needs.
- Fine motor skills sufficient for legible writing, typing, and required documentation.
- Ability to bend, reach, and lift up to 25 pounds while assisting individuals and carrying records or equipment.
- Ability to travel throughout the service area as required.
- Ability to successfully complete all required physical and safety training.
