Service Coordinator
Location: Lufkin, TX
Position Number: P#50
Compensation: $42,120 - $43,555 annually, depending on qualifications and experience
Work Schedule: Monday - Friday, 8:00 a.m. - 5:00 p.m.; after-hours work may be required based on individual and program needs
About the Role
The Service Coordinator provides ongoing direct support to individuals served through General Revenue Programs and the Community First Choice Waiver. This position helps coordinate services and supports that meet each individual's needs while promoting independence and access to appropriate community resources.
The Service Coordinator works closely with individuals, families/LARs, team members, community resources, and internal and external providers. Regular travel is required to provide monitoring services in individuals' homes, schools, and/or day service environments.
What You'll Do
- Complete assessments and required documentation.
- Facilitate program planning and development.
- Monitor services to ensure individual needs are being addressed.
- Link and refer individuals to appropriate services and community resources.
- Provide crisis prevention and management support.
- Develop an understanding of reimbursement practices and program guidelines.
- Communicate and collaborate with individuals, families/LARs, team members, and community resources.
- Work with internal and external providers to coordinate services and support Burke's mission of "Working Together to Improve Lives."
- Travel to individuals' homes, schools, and/or day service environments to provide monitoring services.
- Maintain accurate and timely documentation while meeting required deadlines.
What You Need
Required
- Bachelor's degree in Social Work, Rehabilitation, Psychology, Medicine, Nursing, Counseling, Sociology, Human Development, Gerontology, Educational Psychology, Criminal Justice, or a social, behavioral, or human services field from an accredited college or university.
- Valid Texas driver's license.
- Reliable transportation.
- Driving record that meets Burke's insurance requirements.
- Proficient and timely documentation skills.
- Good typing skills and working knowledge of Microsoft Word.
- Strong organizational skills with the ability to meet strict deadlines.
- Ability to successfully complete required training, including Satori Alternatives to Managing Aggression (SAMA), CPR, First Aid, Defensive Driving, and other required training.

Physical Requirements
This position requires visual and auditory skills sufficient to evaluate and monitor individual needs, fine motor skills for legible writing and typing, and mobility to bend and reach. Must be able to lift up to 25 pounds while assisting individuals and carrying charts and equipment during the workday.
