The PT Vocational Specialist will oversee the daily management of clients, providing support in skills development, and socialization training. The Coordinated Specialty Care program is a specific team-based program designed to provide early intervention services for individuals with first episode psychosis between the ages of 15 to 30. The Mental Health Clinician will provide direct care Supported Employment, Supported Education, and Case Management services to enrolled adults with chronic mental illness in the community/home setting.
This position provides mental health and substance abuse therapy, case management, skills and support services to assist eligible individuals (15 years of age to 30 years of age at STEP admission) in gaining community integration, increase community tenure and maintain the individual's quality of life. The Vocational Coordinator is responsible for managing billing for Individualized Skills & Socialization programs, including maintaining daily logs, processing payments from external providers, and documenting staff hours and contract work. This role also ensures the completion of the annual staff workbooks required for contract eligibility and actively participates in planning meetings to support client care. Additionally, the coordinator supports the daily management of clients by assisting with skill development, socialization training, activities of daily living (such as meal monitoring and medication administration), and promoting community integration. The Vocational Coordinator will serve both the Angelina and Nacogdoches Burke Industries facilities. Other duties as assigned.
The Service Coordinator is responsible for the on-going direct support to individuals served in General Revenue Programs and in the Community First Choice Waiver. Duties include but are not limited to completing assessments, documentation, facilitating program planning and development; monitoring of services; linking and referring individuals to appropriate services; and crisis prevention and management. The Service Coordinator will develop an understanding of reimbursement practices and program guidelines. This position will interact with the individual’s family/LAR, team members, and community resources. The Service Coordinator will also be responsible for working with external and internal providers to accomplish Burke’s mission “Working Together to Improve Lives. “Travel is required to provide monitoring services in the individual’s home, school, and/or day service environment. Self-discipline, flexibility, and time-organization skills are a must.
This position delivers direct physical therapy and consultative services, as recommended by the interdisciplinary team, to developmentally delayed and/or medically at-risk children from birth to age three in a community-based setting. In accordance with Texas licensure requirements, Physical Therapist Assistants must work under the supervision of a licensed Physical Therapist.
