Provider Care Coordinator - K.M. - Developmental Disabilities Provider - Lufkin, TX
The Provider Care Coordinator provides on-going direct service support for people with developmental disabilities for the purpose of providing service coordination involving their natural support networks, such as family members, service staff and other community resources. In addition, the Provider Care Coordinator is responsible for coordinating the development and implementation of each consumer's Person Directed Plan; coordinating the delivery of each consumer's Individual Plan of Care; coordinating and monitoring the delivery of waiver and generic services; integrating various aspects of services delivered under the waiver and through other sources; recording each person's progress; pre-discharge plan development; record keeping and arranging transportation.
Required: Bachelor's Degree from an accredited college or university in Social Work, Social Rehabilitation, Psychology or other Human Services-Related field; OR four (4) years of direct care experience working with IDD.
Required: Four (4) years of direct care experience working with IDD.
Required: Valid Texas driver's license.
Required: Must have visual and auditory skills sufficient to evaluate and monitor consumer needs, mobility required to bend, reach, lift up to 50 pounds while assisting consumers. Must be able to complete SAMA, CPR, First Aid, Back Safety, Defensive Driving and Van Driving. Must be able to carry charts and equipment during a workday. Fine motor skills for legible writing and driving of a motor vehicle for transportation of consumers, business-related travel. Hazardous conditions exist due to potentially aggressive consumers.
Monday - Friday; Generally, 8:00 a.m. - 5:00 p.m.
$27,700 - $30,700 annually, depending on qualifications and experience.
